Periodic Reevaluation & Supportive Periodontal Therapy (SPT)

  1. After initial therapy professional intervention may be needed, what steps are taken before intervention? Scale & root plane, OHI, risk assessment of further disease activity.
  2. Some of the key points in periodontal reevaluation follow, describe their function:

·        BOP: is the best clinical sign something is wrong, but no BOP is helpful in determining if we have arrested the disease.

·        Radiographs: gutta percha may be placed and x-rays taken to measure bone loss.

·        Site by site assessment: must check each site: Gingival BOP= gingivitis, pocket BOP= perio.

·        Microbiological testing: used if patient is not responding to treatment. Paper points are used.

  1. Compliance: Is the patient able to care for their OH as they need to, are they performing the required tasks to help the disease process heal.
  2. Why is it necessary to customize self-care plans for the patient? Not all infected sites are the same, even in the same persons mouth. Certain areas may need certain tools (OH aids) to be able to clean.
  3. How often should a perio patient come in for recall? 6 months is a convience for patient as the insurance pays twice a year. Ideally, perio patients should be seen every 3 months, but once again, the recall should be customized to the patient.
  4. If a patient has refractory perio what does this mean? They are not responding to treatment.
  5. What are some reasons a patient may be refractory? They may have a systemic disease that interferes with the healing process such as diabetes.
  6. Genetic testing: PST is not a diagnostic test.
  7. What might you do for a patient with refractory perio? Prescribe antibiotics, behavior modifications, genetic testing, refer to periodontist if they are not already seeing one.
